Dana Underwood
Vice President
As a healthcare operations leader, Dana brings 10+ years of progressive experience in hospital administration with strong principles associated with strategic planning, physician practice operations, and healthcare information systems. Dana has strong leadership skills and an extensive knowledge in developing services, both professional and ancillary, as well as start-up operations of new practices and offices. She recently served as Director of Operations with Optum, part of UnitedHealth Group for the Denver market. She also served as Senior Administrator for Women’s Health with MedStar Health at MedStar Georgetown University Hospital in Washington, DC, Business Manager with Centura Health at St. Anthony Hospital in Lakewood, CO and Program Coordinator at Denver Health in Denver, CO.
Dana earned her Bachelor of Arts degree in English Studies from Old Dominion University in Norfolk, VA and her MBA in Health Administration from the University of Colorado, Denver.
Outside of work, Dana enjoys spending time with her family, hiking in the mountains, arts/crafts, catching up on Netflix shows, and traveling.
Crista Keller, MD
Chief Medical Officer
Dr. Crista Keller is an internal medicine physician with a clinical focus in geriatric medicine. She has practiced with Aspen Medical Group at Rose Medical Center in Denver, Colorado since 2006. Early in her career, she developed an interest in process improvement related to organizational, operational, and financial aspects of medicine. In 2018, she created a mentoring program for primary care providers in HCA, focusing on these “non-medical” aspects of healthcare. Through mentoring, educational seminars, and leadership roles in HCA, she serves as an advocate for primary care and works to improve communication between clinicians and administrators.
In 2024, Dr Keller joined Colorado Care Partners as Chief Medical Officer, where she collaborates with primary care providers to support the implementation of value-based care models. She provides education regarding coding and clinical process improvement to help providers succeed in current financial models of healthcare. Dr Keller also works with corporate leadership to advocate for primary care and support meaningful interventions to improve care delivery.
Dr. Keller completed her undergraduate education at Coe College in 1996 with a degree in psychology. She completed her medical training at the University of Nebraska and her residency at the University of Arizona, then served as Chief Resident prior to entering medical practice. She worked as a hospitalist in Tucson, Arizona and in Denver prior to joining HCA Healthcare. She continues to work as a practicing physician in addition to her roles as CMO for Colorado Care Partners and mentor for primary care physicians and residents.
Outside of her professional life, Dr. Keller enjoys spending time with her husband and dogs, and with friends and family. She enjoys exercise, spending time in the mountains, gardening, baking, and playing violin. A seasoned world traveler, she loves to share stories from her travels. Additionally, Dr Keller enjoys supporting her husband’s work promoting cardiology care in Southern Africa.
Dr. Keller is deeply committed to improving healthcare through education and process improvement. Her goal is to help healthcare providers optimize their time and energy so they can focus on delivering high-quality patient care while maintaining their personal well-being.
Mohammad Dahleh
Director of Operations and Finance
Mohammad Dahleh is a senior healthcare finance and administrative leader with more than 10 years of progressive experience in academic medicine and public health systems. He most recently served as business services program director at the University of Colorado Anschutz Medical Campus, where he led finance and administrative operations for complex clinical, research and academic portfolios.
He brings expertise in financial management, revenue cycle optimization, provider compensation design and productivity benchmarking. Mohammad is passionate about strengthening the financial infrastructure that enables clinicians and care teams to deliver high-quality, equitable healthcare.
He holds a Master of Science in accounting and dual bachelor's degrees in accounting and finance. Mohammad is committed to advancing financial sustainability, operational excellence and compassionate healthcare delivery to ensure long-term organizational resilience and community impact.
Macklin Nickles
Director of Population Health
Macklin Nickles interest in value-based care started while completing his Master of Health Administration from the University of Memphis. As an administrative resident at Baptist Memorial Healthcare in Memphis, Tennessee he focused on contracting and financial analysis in a fully capitated model. Prior to joining Colorado Care Partners, he managed a dual primary care and urgent care office in Castle Pines, Colorado, and then joined HCA Healthcare supervising business office staff for three multi-specialty surgery centers in the greater Denver area.
Originally from Arkansas, being outdoors has always been a major part of Macklin’s life. He enjoys camping, spending days in Denver’s great parks and snowboarding in the winter. He also volunteers for Downtown Denver’s Business Improvement District – the Urban Forest Initiative.
Cindy Chumacas
Director of Business Development
Cindy’s role as Director of Business Development is being responsible for supporting CCP in business development, overall strategic growth, marketing, physician relations, and community outreach. She is also responsible for creating and maintaining positive relationships with current practices, providers, members, and payors as needed for identifying, building, and maintaining key partnerships in order to achieve a successful collaboration.
